Being new to Fallen London … I keep seeing " spend a _______ to ensure a second chance " pop up. Does this mean that if I don’t spend the item that that action / choice will never pop up again?
nope. it just prevents you for failing the action, at least once. the consequence of failing might vary, so, if it’s a storylet, you can just do it again (but you might lose an item, or get some menaces when you fail). if it’s an opportunity card, iirc, you lose it, and you have to draw it again.

You can forgo the re-roll, by the way; you sitll lose the Action Point, but you don’t suffer the consequences for failing the roll.
Re-rolls for that particular Action will cost far more Advantages next time though. That debt won’t go away.
