What are the differences between one patron and the other story-wise? And how do they affect the storylets I am able to choose based on that?

Nobody knows. The storyline caps after that, with the only difference being the reward you get when the Living Story appears in your messages.

At present, the story ends almost as soon as one has chosen a patron, and the choice has no practical effects. Presumably, the choice will become important once more content is added. Story-wise, we learn that His Amused Lordship wants to use the club's discoveries to benefit all mankind; the Implacable Detective wants to keep them for those who need them most; and the Bishop wants to put them out of reach of humanity.
