Is there some special trigger for the "courier for the dead" card or is it just really really rare?

Maybe the random number generator just isn't smiling upon you this evening. I don't believe there is a special trigger, but I also don't believe that the card is "really really rare" - uncommon, perhaps, but I've died a number of times and it usually turns up.
