If your activated day is Sunday, time the healer will arrive Sunday or Monday next week?
Changing your profession will cause time the healer changing as well?

1. It should arrive on the day of the activation but I swear to God mine moved one day ahead after three-four months of playing.
2. It's not connected with career changes.

The only thing that changes when Time the Healer arrives is server troubles. However if server troubles delay your TtH, the new, later time becomes permanent.

You do not need a profession for TtH to arrive, you only need one to claim your payment. In fact, after it arrives you can switch your profession and take a different pay-out. It simply grants the "Earnest of Payment" quality, which unlocks the payment storylets.

From the horse's mouth after my TTH moved by a couple of days in one go: It (also) happens when a whole swathe of new "lives" are established in the Neath.
