So I've got this mysterious item. Does this mean that I'm committed to using it or tossing it into the sea when I return to London, or can I make the decision whenever I like? And if I do toss it, can I get another one?

If you go to London and hand it over to the Anarchists, basicaly everything goes to %^&$ and London will no longer be accessible to you. When you get to London you can instead throw it away after retrieving 7 Game Moves and 1 Whispered Secret. No, you cannot get another one.
