Maybe the random number generator just isn’t smiling upon you this evening. I don’t believe there is a special trigger, but I also don’t believe that the card is “really really rare” - uncommon, perhaps, but I’ve died a number of times and it usually turns up.
Are you sure? Its been a while…
Perhaps they’ve change it since the last time I was there. They have been rolling out changes rather quickly.
